Patient Information in firstREV

To view details for a patient:

1     Using the toolbar, click Patients and then select Patients.

2     Find the patient whose details you want to view. If necessary, you can use the Quick Search menu to search for Patient Name, Patient ID, Patient DOB, or Payer Name.

3     Select the patient by clicking on the corresponding record in the grid or by clicking the Edit icon ( ) in the Action column. The Patient Folder screen will display for the selected patient.

4     As seen in the sample image, users can access a variety of different types of information for the selected patient. Information is split up into different categorized tabs (Overview, Transactions, Payment Estimates, Notes). Learn more about the Patient Information tabs below.

 

firstREV groups all information in the Patient Folder into several different patient information tabs:

1     Overview — View existing general information about the patient (i.e. Provider, Addresses, Demographics, Insurance ).

2     Transactions — Displays all claim and remittance entries associated with the selected patient. Each entry includes the following information:

A     Account Number

B     Payer

C     Statement Date

D    TOB (Type of Bill)

E      Charge

F      Payment Date

G    Payment Amount

H    Status

I        ICN

J       Action — This column includes a View Link icon that allows users to view the corresponding claim or remittance file.

3     Payment Estimates — Allows users to calculate an actual estimate of how much a patient may have to pay for requested care, which is dependent upon the patient’s insurance. After entering all application information in the fields, users can click    to get an estimate. Users can also click    to save the estimate, which will then be located as an entry in the Saved Estimates section at the top of this tab.

As seen in the sample image, users have the choice to Load the estimate into the current screen, Print the estimate (into a .pdf file), or Delete the saved estimate.

4     Notes (#) — The Notes tab is an internal feature that can be used with discretion to store patient-specific information within the firstREV interface. Click  , enter the details for the note, and then click Save to store a new note within this tab (for the selected patient). Users can also view or remove existing patient notes. The tab header itself also indicates the number of available notes for the selected patient (if applicable).

 

The Patient Folder screen also contains two control tabs that users can access at any time while viewing any of the patient information tabs.

 
 
   

Creating New Claims

While viewing any screen within the Patient Folder, users can create a new claim for the selected patient. Users have the option to create a new blank claim or create one from a copy of a previous claim.

 

To create a blank claim:

1     If necessary, navigate to Patients | Patients and access the Patient Folder for the desired patient.

2     Hover the mouse cursor over the Patient tab and click New Claim. The New Claim prompt displays.

3     In the Create Claim section, click the radio button next to Create Blank Claim.

4     In the Claim Type section, use the drop-down menu to select either Professional or Institutional.

5     In the Select Insurance section, designate payer(s) for services included in the claim. Any payers associated with the patient will display in this list. Use the drop-down menu in the Sequence column to select the appropriate option for payer(s) listed in this section (i.e. Primary, Secondary, Tertiary, -Not Selected-).

6     In the Services section, complete all fields to enter details for services in the new claim (Date, CPT/HCPCS, Description, Charge, Units ).

7     Click Create. If desired, you can choose to view the new claim immediately.

 

To create a new claim from a previous claim:

1     If necessary, navigate to Patients | Patients and access the Patient Folder for the desired patient.

2     Hover the mouse cursor over the Patient tab and click New Claim. The New Claim prompt displays.

3     In the Select Claim section, click the corresponding radio button to specify the existing claim you want to use to create the new claim.

4     Enter the New Service Date.

5     Click Create. If desired, you can choose to view the new claim immediately.

Printing Patient Information

In the Patient Folder screen, users also have the option to print patient information by hovering the mouse cursor over the Print tab and then selecting either Summary or Statement.

 

Patient Summary

Choosing this option will automatically generate a .pdf file with a brief synopsis of certain information about the selected patient. Simply click the link in the confirmation prompt that displays to download the .pdf file. This summarized patient information file includes:

1     Name

2     Date of Birth

3     Gender

4     Insurance information

A     Payer

B     Subscriber

C     Relationship

D    Member ID

E      Status

 

Patient Statement  

When Statement is selected, a Patient Statement prompt will dis play (See sample image), allowing users to specify which criteria is used in the patient statement to match organization standards.

Click Run Report to generate a .pdf file that includes all the specified information for the selected patient. Click the link in the confirmation prompts that displays to download the .pdf file.
