Use the Custom Form Images tab to upload images you want to use within Custom Forms. Examples of images may include body image for wound markers, illustrative pain scale, or agency logos. If necessary, you can add the ability to display a marker on the image. This is useful for indicating wound locations or other items on the image itself.
Uploaded images must meet the following criteria:
● File format: jpeg, jpg
● Size: 2MB
To upload a custom image:
1. Ensure you save your image to a location from which you can upload.
2. Click .
3. Click Browse to locate where you saved the image.
4. Click on the image file, and click Open.
5. Edit the Image Name (the file name defaults as the image name). The Image Name must contain ".jpg" at the end of the name.
6. Add a Tool Tip if necessary. The tool tip displays when the user moves the mouse over the image.
7. Select a Range Start and Range End for image markers, if necessary.
8. Click . The image displays at the bottom of the window after the virus scan is complete.
9. If your image uploaded correctly, click Publish to make the image available to add to a custom element or custom form.
