Patient Identified Strengths / Goals / Preferences

Agencies can utilize the Strengths / Goals / Prefs area located in Patients | Intake / Referral to track additional patient information pertaining to strengths, goals, and preferences that have been identified for the corresponding patient. To comply with the Conditions of Participation requirements mandated by CMS, information in this screen will synchronize with home health care plans and comprehensive documentation. Users can enter information pertaining to patient strengths, goals, and preferences in this section or within applicable Homecare documents (using the Actions feature in the appropriate section). The software will synchronize data in this screen with the following Homecare documents.

 

Users are able to enter a variety of different information for patients in the Patient Identified Strengths / Goals / Preferences screen.

     Patient Identified Strengths — Check the applicable boxes to indicate the corresponding options that apply to the selected patient.

     Patient Goals — Enter the description of a patient goal within the appropriate text field (500 character limit) that is available.

For home health agencies: If the Care Planning option is set to "on" for an agency's office (in Administration | System Control), the Patient Goals section will be disabled and users cannot edit any of the fields. This ensures that care plan information is consistent and users are only able to edit a patient's care plan by using the proper methods.

     Patient Preferences — Use the appropriate checkboxes and/or text fields to indicate specific preferences that the patient has regarding their methods of care.
