Advance Directives

To add an advance directive under Reference Files | Advance Directive:

1.    Click  .

2.    Enter the Directive.

3.    Use the Directive Type/Value Based Purchasing Mapping field to specify the type of advance directive, which is necessary to determine CMS VBP responses (as required for states participating in the new Value-Based Purchasing payment model).

Though this field is only required for agencies in states that are participating in the new Value-Based Purchasing payment model, all firstHOMECARE agencies can elect to use this feature (if desired).

4.    Check the Include in Assessments box to enable the software to synchronize the corresponding advance directives in comprehensive documentation (e.g. Start of Care, Resumption of Care, Follow-up, Recertification). If this box is not checked, the software will not populate data for the corresponding directive in comprehensive documents for all patients.

5.    Click  .

To update an advance directive under Reference Files | Advance Directives:

1.    Click on the advance directive you want to update. You can search for the advance directive by entering the Directive and clicking .

2.    Make any necessary changes to the appropriate fields for the advance directive (i.e. Directive, Directive Type/Value Based Purchasing Mapping , Include in Assessments ).

3.    Ensure that existing directives are assigned with the appropriate value in the Directive Type/Value Based Purchasing Mapping field, which is necessary to determine CMS VBP responses (for agencies participating in VBP). By default, this field will contain a blank status for all existing directives until changed by a user..
