Enhancements
The Hospice Quality Reporting Program (HQRP) is a mandate of the Affordable Care Act of 2010. Follow the link below for specific details:
http://www.hospicecahpssurvey.org/content/homepage.aspx
As part of HQRP, the Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey will be a requirement for all hospices. The survey is a standardized experience of
care survey designed to measure the experiences of patients who died while receiving hospice care, and considers the hospice patient and his/her primary informal caregiver as the unit of care.
The survey is administered to decedents/caregivers from all payor sources. Providers are required to participate in a Dry Run for at least one month in the first quarter of 2015 (January, February, or March), and then will be required to participate on a continuous monthly basis beginning April of 2015.
CAHPS Patient Eligibility
Patients eligible for inclusion in the CAHPS Survey:
Decedents that are 18 years old and older.
Decedents that passed away at least 48 hours following the last admission to hospice care
Decedents for whom there is a caregiver record (Patients without caregivers will be included in the hospice CAHPS file and the CAHPS vendor will filter them out prior to submitting data to CMS)
Decedents whose caregiver is someone other than a non-familial legal guardian (Patients with a non-familial legal guardian/relationship type #6, will be included in the hospice CAHPS file and the CAHPS vendor will filter them out prior to submitting data to CMS)
Decedents for whom the caregiver has a U.S. or U.S. Territory home address (No address filtering will be performed. Hospice CAHPS vendors will filter out any necessary records prior to submitting to CMS)
Patients not eligible for inclusion:
Decedents/caregivers who requested they not be contacted (those who signed a "no publicity" request while under the care of hospice or directly request not to be contacted).
Patients whose last admission to hospice resulted in a live discharge.
Changes to Enterprise Edition
Changes have been made to Reports | CAHPS Survey to add a new CAHPS Survey that will create a comma-separated value (.csv) file that includes the data elements required by CMS regulations.
Changes have been made to Patients | Intake Referral | Demographics to add a Veteran checkbox. The checkbox will not be selected by default. Click the box to select and check it if the patient is a veteran to meet Hospice CAHPS requirements.
Changes have been made to Patients | Intake Referral | Patient Admission and Family Caregiver to add an Exclude from CAHPS checkbox. The checkbox will not be selected by default. Click the box to select and check it if the patient needs to be excluded from the CAHPS report.
Hospice exclusions differ from homecare exclusions. Patients will only be excluded from the CAHPS report if the Exclude from CAHPS checkbox is selected. Following are the reasons for hospice exclusion:
Expired Less than 48 hours = Patient expired less than 48 hours following the last admission to hospice care. (Enterprise Edition determines that a patient is deceased based on the patient status. If a patient is expired, ensure that the expired checkbox is selected and the patient status has been changed.)
Live Discharge = Patient status is something other than 20, 40, 41 and 42.
No Publicity = Caregiver has “Exclude from CAHPS”
Under 18 years old = Patient is less than 18 years old during the sample month.
Creating the CAHPS Export File
To be in compliance with the CMS HQRP mandate, you will need to create, export, and submit the CAHPS report on a monthly basis after the initial first quarter dry run period.

7. Select the applicable Report Options from the drop-down menu.
8. Click the box next to Expanded Export to select and check it if you need to run the Expanded report version for your vendor.

The CAHPS export file will be in the following format:
Comma separated value (CSV) format.
Fields with embedded commas will be double quoted.
If data for a specific column is missing or unknown, it will be left blank or marked as missing with ‘M’.
NOTE: HEALTHCAREfirst provides two export formats, Basic and Expanded. The only difference between the two formats is that the Expanded file includes two client identifier fields necessary for specific CAHPS vendors that also provide CAHPS for Home Health in addition to Hospice.
Any CMS-approved contracted CAHPS vendor should be able to accept and use the file generated. HEALTHCAREfirst supports a number of CAHPS vendors. View the following link to see if your CAHPS vendor is supported.

HEALTHCAREfirst recommends sending a copy of the Export File Requirements, which can be found in the Regulatory Update 10005 (contact Support if you do not have a copy of this document), to your CAHPS vendor so they can make any necessary adjustments to the import for the file to be accepted. Please note, if your vendor requires any additional data other than the CMS required elements, you will be responsible for adding this information to the file.
HEALTHCAREfirst recommends contacting your contracted CAHPS vendor to determine the process for uploading and submitting data. Most CAHPS vendors offer a secure upload feature/site.
