Admissions With Missing Diagnosis Report

This report provides a list of patients that contain diagnoses that are either missing a corresponding ICD-9 or ICD-10 code. This report was created to provide users with a comprehensive list of all patient diagnoses that have ICD-9 codes without ICD-10 codes "crosswalked" (matched and saved to patient chart), and vice versa, to assist in the prevention of rejected or incorrect claims.

Select the criteria you want to use to filter the report:

1.    To show only active admissions in the report, click the Active Only check box.

2.    Change the following fields if you want to filter by specific items other than the default settings:

     The Patient Name From and Patient Name Thru fields contain All to include all patients in the report. To filter the report by a specific patient, or a range of patient names, enter the range of names in these  fields.

     The Active Patients From and Active Patients Thru dates include all active patients up to the current date. To filter by any other dates, click in the these fields and select the appropriate dates from the calendars.

3.    To only display patients that are missing an ICD-10 code from their chart, leave the ICD10 is Missing box checked. To display patients that are missing an ICD-9 code from their chart, check the box next to ICD09 is Missing.

4.    For a more simplified version of this list, which merely groups the office numbers with the patient names that are missing ICD codes, click the box next to Suppress Details.

By default, this report includes the following columns:

  • ICD-10 Code

  • ICD-10 Description

  • ICD-09 Code

  • ICD-09 Description

  • Primary (Indicates whether primary diagnosis)

  • Dia Seq

5.    Click the arrow beside the Offices field, and then select the check box beside the office you want to use. To see all offices, click the top-level check box.
