Creating CAHPS Survey

The Consumer Assessment of Health Providers & Systems (CAHPS) is a program of the U.S. Agency for Healthcare Research and Quality (AHRQ) and is used to survey consumers and patients to report on and evaluate their experiences with health care.

Use the CAHPS Survey report to view responses from the survey and evaluate the quality of your agency's healthcare services. Select the criteria you want to use to filter the report

4. Click the arrow beside the Offices field, and then select the check box beside the office you want to use. To see all offices, click the top-level check box.

5. To show only responses for Medicare and Medicaid, click the MCR/MCD Only check box.

6. Click the arrow beside the Month field and select the month containing the surveys you want to view.

8. Click the arrow beside the Report Options field and select one of the following options:

9. If you know the Survey Designator and/or the Vendor Client ID, select the Expanded Export check box and enter the appropriate information. (These options will be unavailable if the Expanded Export check box is not selected.)

The Home Health CAHPS Export Report contains the following information:

● Header information:

o Date

o Offices

o Export Period

o Export Options

● Report columns — The report displays the following information from the CAHPS Survey:

o Patient Name

o ID

● The following columns contain an X beside the patient if it applies to that patient:

o Not MCR/MCD

o Deceased

o Excluded

o Not Enough Prior Visits

o No Skilled Visits

o Under 18 yrs old

o Incomplete Assessment

o Missing Diagnosis

If you want to export the report:

1. Click the arrow beside Select a format at the top of the report window, and select whether you want to report to Excel, a CSV file, Acrobat, or Word.

2. Click Export CAHPS at the top of the report.

3. Enterprise Edition formats the report based on your selection and downloads it to your computer's default location.
