This report provides a listing of all insurance payors and the associated plans, provider numbers, and addresses. Select the criteria you want to use to filter the report:
1. Change the following fields if you want to filter by specific items other than the default settings:
● The Payor ID From and Payor ID Thru fields include all payor IDs. To filter the report by a specific ID, or a range of IDs, enter the range of IDs in these fields.
● The Payor Name From and Payor Name Thru fields contain All Payors to include all payors in the report. To filter the report by a specific payor name, or a range of payor names, enter the range of names in these fields.
● The Plan Name From and Plan Name Thru fields contain All to include all plan names associated with the selected payors in the report. To filter the report by a specific plan, or a range of plans, enter the range of names in these fields.
2. To show all details in the report, click the Show Details check box. Otherwise, clear this check box to see a high-level report without details.
3. Click Preview.
