This report provides a listing of physicians, their name, specialty, office #, fax #, email address, registration #, and expiration date. Select the criteria you want to use to filter the report:
1. Click the arrow beside the Offices field, and then select the check box beside the office you want to use. To see all offices, click the top-level check box.
2. To show only active physicians in the report, click the Active Only check box. Otherwise, clear this check box to see a high-level report listing all physicians.
3. Change the following fields if you want to filter by specific items other than the default settings:
● The Physician ID From and Physician ID Thru fields include all physicians. To filter the report by a specific ID, or a range of IDs, enter the range of IDs in these fields.
● The Physician Name From and Physician Name Thru fields contain All to include all physicians in the report. To filter the report by a specific physician name, or a range of physician names, enter the range of names in these fields.
● The Expiration From and Expiration Thru fields include ID expiration dates. To filter the report by a specific range of expiration dates, enter the range of dates in these fields.
4. Click Preview.
