1. From the Main Menu, click Tools then select OASIS Extract.
2. Click Header Info.
3. Highlight the office to edit.
4. Edit any applicable fields.

Use the OASIS (Outcome and Assessment Information Set) Extract to extract files, create submission files, and correct OASIS assessments previously submitted to the state.
Every OASIS assessment includes basic information in a header that identifies the agency submitting the OASIS assessment. While you only need to enter this information one time from each office, you must enter it prior to submitting the first OASIS Assessment to the state from your agency.
You can update the OASIS header information at any time. Once you update the header information, it affects the OASIS assessments you submit from that point forward.
