Tracking Patient Medical History
firstHOSPICE agencies are able to use the screen located in Patients | Intake / Referral | Medical History to track various information that is related to the patient's medical history. Similar to most screens throughout the software interface, users must click
to save any changes that are made to existing data.

All data in the Medical History screen automatically synchronizes with the "History" page in documents that are associated with the corresponding patients. This ensures that all of the sections of data (listed below) will display the most current information for the corresponding patient.

The following information can be tracked for patients in the Patient Medical History screen:
● Patient Information
● History of Diagnosis / Disease— Indicate any diagnosis or disease in the list that applies to the patient (e.g. Alzheimer's disease, Diabetes, Cirrhosis, Mental/Psych Disorders, Vascular Disease, etc.) as well as the applicable timeframe for the corresponding diagnosis/disease (i.e. Less than 6 mo., 6 mo. - 1 yr., 1 - 5 yrs., or Greater than 5 yrs.)
● History of Cancer(i.e. Type of Cancer, Date of Diagnosis, Biopsy information, Results, Treatment information, etc.)
● Surgeries / Hospitalizations— Indicate any surgeries and/or hospitalizations that have occurred in the last 3 months, 3 - 6 months ago, or greater than 6 months ago. Additionally, include any details regarding complications that were encountered from surgeries.
● Functional Ability— Track medical information regarding the patient's Overall Ability over time, their Ability to perform 3 months ago for different activities, as well as their Ability to perform 6 months ago for the same activities.
● Body Systems— Track various medical data pertaining to body systems for the patient (i.e. Weight, Nutritional Status, % of Incontinence, Skin Breakdown / Wounds, Sleep, Musculoskeletal Condition ).
