Bereavement Listing Report

This report provides hospice agencies with a direct resource for bereavement information. While all versions contain an overview of the bereavement status for applicable patients, users can also view a more detailed version with additional information for patients and patient contacts. Patient contacts will not be considered for this report if No caregiver involved in the care of the patient is selected in Patients | Intake / Referral | Patient Contacts.

 

Select the criteria you want to use to filter the report:

1.    Use the Offices drop-down field to indicate the hospice office(s) to use to gather bereavement data. To use all offices, select the top-level checkbox.

2.    Specify the date range for viewing applicable report data using the Date From and Thru fields.

The report will consider these dates to include applicable data for the statuses of Admitted, Discharged, Declined, and Not Admitted. Each of these statuses correlates with a specific date field. This enables the report to include statuses based on the date entered in the corresponding field, as well as the date range identified in the report filter.

3.    Filter report data according to the bereavement status of patient contacts using the Bereavement Status field. The status of each contact coincides with certain fields in Patient Contacts or Bereavement.

     All

     Admitted

     Discharged  

     Declined

     Pending

     Not Admitted

     Blank

4.    Specify the type of bereaved entries to display in the report using the Bereaved Type field.

     AllInclude all types of bereavement.

     Hospice Bereaved OnlyOnly include contacts that are not community bereaved.

     Community Bereaved OnlyOnly include bereaved contacts that have the Community Bereavement box checked in Bereavement | Deceased.

5.    Check the Show Details box to include additional bereavement data beyond the standard Bereavement Status summary section. By default, this box is not checked.

6.    Check the Bereaved Active Only box to include any patient contacts that are currently active in Bereavement (Admit Date is present, but Discharge date is not) and do not have a pending, blank, or declined status. By default, this box is not checked.
