This report allows hospice agencies to audit bereaved patient contacts for required information that is missing (in Patients | Intake / Referral | Patient Contacts). Users can run this report to determine missing information instead of manually verifying fields by navigating to Patient Contacts. This ensures a smooth bereavement process as users can easily verify that everything is completed prior to discharging the patient.

This report will only include active patients if they have a contact that has a Bereavement Status of "Pending Bereavement" and is missing at least one of the required fields. |
Select the criteria you want to use to filter the report:
1. Specify the date range for viewing applicable report data using the Date From and Thru fields.

If a date range is entered, the report will include all patients that are still active within the date range (a discharge date is not present), have a contact with a Bereavement Status of “Pending Bereavement”, and one or more of the required fields are missing. This ensures that contacts who are “Pending Bereavement” for active patients have all of the necessary information completed prior to patient discharge. Once a patient has a discharge date, they will no longer populate in this report for any date range — as all fields will be completed due to the new warnings that have been implemented. If a patient was discharged during the date range or discharged/admitted after the date range, they will not be included in the report data. If a patient was admitted prior to this date range, is still active during the date range, and does not have a discharge date present when running the report, they will be included in the report. |
2. Use the Offices drop-down field to indicate the hospice office(s) to use to gather bereavement data. To use all offices, select the top-level checkbox.
