Although this report was originally created to assist California agencies with ALIRTS, this report can be used for other purposes by agencies in other states.
Home Health and Hospice agencies operating in the state of California are required to complete an annual utilization report through the Automated Licensing Information and Report Tracking System (ALIRTS). To assist users in gathering the information that is necessary for this utilization form, HEALTHCAREfirst has provided the ALIRTS Report. The software will only consider patient data for each section if they have a visit in Productivity.

This report can be used to gather information that is necessary for the ALIRTS utilization form:
1. Click the arrow beside the Offices field and check the boxes to select the office(s) you want to use. To see all offices, click the top-level check box.
2. To view a detailed version of the report, select the Show Details checkbox. This version of the report will result in a significantly larger number of pages.
3. If desired, use the Service From and Thru fields to filter date according to a specific date range. By default, the previous year is selected (e.g. 1/1/2017 to 12/31/2017).
4. Click Preview. Depending on what type of browser you use, you may see a message asking if you want to open or save the file. If you select the save option, the system prompts you to select a location where you want to save and name the file.

Understanding the report content
The Hospice ALIRTS Report contains the following sets of data:
Bereavement Services: Total number of survivors of hospice patients served and the total number of survivors of persons not receiving hospice care served within the report period.
● Each hospice patient served with a visit in the Productivity screen (billable/non-billable and covered/non-covered) – excluding supplies – that falls within the report period is included in the report.
● Survivors of hospice patients that are not receiving hospice care will be included in the totals if Community Bereavement is checked in Bereavement | Deceased and the bereavement chart has an admission date within the report period.
Visits By Type of Staff: Total number of visits conducted by each type of staff (personnel type) within the report period.
● Displays all personnel types in Reference Files | Personnel Types.
● Any visit in Productivity (billable/non-billable and covered/non-covered) – excluding supplies – that falls within the report period is included in the report.
● For each visit, the type is determined by the staff assigned to the visit and the Personnel Type selected in Personnel | Employment.
Unduplicated Hospice Patients By Gender and Age: Total number of patients by gender for each age group that has received service from the agency within the report period.
● Patient with a visit in the Productivity screen (billable/non-billable and covered/non-covered) that falls within the report period will be included in the totals.
● Age is determined based on the age of the patient at the time of admission
Unduplicated Hospice Patients By Gender and Race: Total number of patients by gender for each race that has received service from the agency within the report period.
● Patient with a visit in the Productivity screen (billable/non-billable and covered/non-covered) that falls within the report period will be included in the totals.
● Race is determined by the Ethnicity selected in the Patients | Demographics screen.
Unduplicated Hospice Patients By Gender and Ethnicity: Total number of patients by gender for each ethnicity that received service from the agency within the report period.
● Patient with a visit in the Productivity screen (billable/non-billable and covered/non-covered) that falls within the report period will be included in the totals.
● Ethnicity is determined by the Ethnicity selected on the Patients | Demographics screen.
Hospice Patients Discharged By Reason: This section will display the total number of discharged patients per each reason for discharge within the report period.
● Lists all discharge reasons with the Hospice box checked in Reference Files | Admit/Dischg Status – excluding statuses “NA”, “Do Not Admit”, and “30 - Still a Patient.”
● Patients will be included in report totals if they have both a visit in the Productivity screen (billable/non-billable and covered/non-covered) and a discharge date that falls within the reporting period.
Hospice Patients Discharged By Length of Stay: Total number of discharged patients for each range of days listed that received service from the agency within the report period.
● Patients will be included in report totals if they have both a visit in the Productivity screen (billable/non-billable and covered/non-covered) and a discharge date that falls within the reporting period.
● Number of days are calculated by taking the difference from the admission date and the discharge date listed in the patient chart.
Hospice Patient Admissions By County and Discharges by Disposition: Total number of admissions by county, discharges by deaths, and non-death per county for patients served within the report period.
● Number of admissions include any patients that have a visit in Productivity and an admission date that falls within the reporting period.
● Number of deaths include patients with a visit in Productivity and a discharge date within the report period and have a discharge status of expired (40, 41, and 42).
● Number of non-death discharges include patients that have a discharge date within the report period and a discharge reason other than “30-Still a Patient”, “40-Expired at Home”, “41-Expired at a medical facility”, or “42-Expired - Place Unknown”.
● Hospice patients served will include each patient that has a visit in the Productivity screen (billable/non-billable and covered/non-covered) within the report period. Each patient is only counted once in this section.
Number of Hospice Admissions By Diagnosis: Total number of admissions, re-admissions previously seen by another hospice program, and re-admissions seen by this hospice program for each diagnosis category listed. This applies to any patients that were admitted and/or re-admitted within the report period.
● New admissions include any patients with a visit in Productivity and an admission date within the report period whose primary diagnosis falls into the associated categories.
● Re-admissions from another hospice include any patient that has a visit in Productivity, was re-admitted with a date that falls within the report period, and has the Transfer from Other Hospice Agency box checked in Patients | Demographics. Patients will only be included if the primary diagnosis falls into one of the associated categories.
● Re-admissions previously seen by this hospice program include any patient that has a visit in Productivity, was re-admitted with a date that falls within the report period, and has the Previous Hospice Election box checked in Patients | Demographics. Patients will only be included if the primary diagnosis falls into one of the associated categories.
Discharged Hospice Patients, Visits and Patient Days By Diagnosis: Total number of live discharges, discharges due to death, discharges, visits, and patients per diagnosis category listed. This applies to patients that were discharged within the report period.
● Number of live discharges include any patient with a visit in Productivity and a discharge date within the report period that has a discharge reason of 01, 50, or 51. Patients are only included if the primary diagnosis falls into one of the associated categories.
● Discharge due to death includes any patient with a visit in Productivity and a discharge date within the report period that has a discharge reason of 40, 41, or 42. Patients are only included if the primary diagnosis falls into one of the associated categories.
● Total number of discharges include patients that has a visit in Productivity and a discharge date within the report period and their primary diagnosis falls into one of the associated categories.
● Visits for discharged displays the total number of visits in Productivity that fall within the reporting period for patients with a discharge date.
● Discharged patients total days of care displays patients with a visit in Productivity as well as the total number of days under all four levels of care (Routine, Inpatient, Inpatient Respite, and Continuous) in the report period.
Level of Care and Source of Payment: Total number of patients served for each level of care and source of payment within the report period.
● Displays all payors with “Hospice” selected as the Line of Business in Master Files | Payors/Plans | Payors.
● Patients served is determined by patients that have both a visit in the Productivity screen (billable/non-billable and covered/non-covered) – excluding supplies – and a discharge date that falls within the report period.
● Total number of days pulls from the Patients | Level of Care screen.
Location of Care Provided: This section will display the total number of patients served for each level of care and location within the report period.
● Displays all locations listed in Reference Files | Location Codes.
● Patients served is determined by patients that have both a visit in the Productivity screen (billable/non-billable and covered/non-covered) – excluding supplies – and a discharge date that falls within the report period.
● Total number of days pulls from Patients | Admission/Discharge | Level/Location of Care.
Hospice Income Statement: Retrieve this information by running the Cost Report in firstHOSPICE.
Level of Care Hospice Sites Provide: For inpatient facilities/units, refer to the Location of Care Provided section for the totals of each level of care for your inpatient office.
Hospice Income Adjustments: This section will display specific data pertaining to hospice income adjustments.
● Table displays information for all hospice income adjustments, sorted by two columns of data:
o Adjustment Type— Lists all adjustment types for the agency, based on records that are saved in Reference Files | Payment/Adj Codes).
o Total— Total dollar amount for the corresponding adjustment type that was posted for claims with dates of service that occurred during the reporting period. The cumulative Total for all adjustment types is listed in the last row of the table.
● Detailed version of the report (Show Details report filter is selected) includes contextual patient information related to the adjustments information displayed in the table above:
o Includes several columns of data.
(Patient Name, Patient ID, Admit Date, Adjustment Type, Adjustment Amount, Batch Date, Remit Number )
o Each item is first grouped according to adjustment type and is then listed alphabetically.
o Totals are included for each set of information.
