Use the Custom Form Elements tab to create a group of fields you can use in a variety of forms to eliminate the need to add each field to each document. Examples of grouped fields are vitals, pain questions, and discipline specific questions.
To create a new custom form element:
1. On the Custom Form Elements tab click .
2. Enter a name in Element Name.
3. Click the arrow beside Line of Business and select the line of business to determine how the element will be available when creating a custom form.
4. Click .
5. Select your element in the list, and click Edit to open the Element Builder.
6. Drag and drop the controls from the left to the yellow box on the right to build your element.
7. Continue with Using the Element and Form Builders to edit the controls.
8. When finished, continue with Saving and Closing the Builders.
