To select the physician(s) associated with a patient in Patients | Intake/Referral | Physicians:
1. Click .
2. Below Physician Search, in the Name field, enter the physician name, or partial name, and click .
You do not need to enter the complete name to search for the physician. |
3. From the list of physicians found, click the physician name to select it. Enterprise Edition will display the physician information below the Physician Details section.
4. In the From field enter the effective date the physician should be associated with the patient. (Enter the effective Thru date when the physician is no longer associated with the patient to assist in the tracking of historical physicians.)
5. Click the box next to Historical to select it when the physician is no longer associated with the patient and needs to be tracked as a historical physician.
Historical Attending and Certifying physicians may not contain overlapping From and Thru dates with any Active physicians. If you attempt to add a Historical physician with the same dates as an existing Attending or Certifying physician you will receive an error message stating "Attending (or Certifying) Physician already set for time frame. Only on is allowed per time frame." |
6. By default, Enterprise Edition selects the Attending check box to indicate this is the patient's attending physician. If this physician is not the attending physician, click the check box to de-select it.
7. If the certifying physician is different than the attending physician, check the Certifying check box. This will include the certifying physician on the UB04 claim, if the certifying physician is different from the attending physician in Enterprise Edition.
8. If the selected physician is a Medical Director or certifying physician, check the Medical Director box.
9. Check the Ordering to indicate that this is the ordering physician for the patient. As the ordering provider is not always the attending physician, this helps agencies avoid data inconsistencies for different circumstances. Checking this box for the ordering physician ensures that the correct physician is listed for applicable service/visit lines in 1500 (02-12) claims that are submitted as long as the Include Ordering Provider box is checked for the payor in Master Files | Payors / Plans | Billing Formats.
If you need to add a new physician or edit an existing physician in the list, click beside the Name field to open the Reference File editor and enter the appropriate information. To return to the physician information, click the Physicians tab. |
10. Click . Enterprise Edition will display the physician information in the Physicians found grid at the top of the window.
11. Click to apply pending changes and continue with Entering Patient Insurance.
Due to the need to track Historical physicians for CMS requirements, physicians should not be deleted except for scenarios where the physician was added to the patient record in error. In the event that you attempt to delete a physician, you will receive an error message stating "Deleting this physician will permanently remove it. To archive the physician for historical use instead, click Cancel and enter an Thru date or click OK to continue deleting."
