Health Screening can be used to enter and track health screening information related to herpes zoster/shingles vaccinations for patients (as required for states participating in the new Value-Based Purchasing payment model). Though this functionality is intended for agencies in VBP states, all firstHOMECARE agencies can elect to use this feature.
If applicable, agencies can also enter this information in comprehensive versions of the Start of Care/Resumption of Care, Recert/Follow-Up, and Discharge OASIS assessments. Agencies that utilize HCF comprehensive OASIS documents only need to enter this information in one of these locations as data will flow between the two locations.

In the case that a patient's vaccination response changes (e.g. Patient initially declines vaccination at the Start of Care and then later requests the vaccine), agencies should simply add a new vaccination record for the response (instead of modifying the previous entry). At any point during a patient admission, users can add a new record to the patient chart if information needs to be updated. In a situation where the patient response was incorrect the first time, users should still edit the existing vaccination record and then add a new vaccination record. The primary purpose of this functionality is to track all true changes made to the patient chart. Be aware that the grid will not display an entry for every corresponding OASIS record in the patient chart — the grid is only updated when a response changes. The only time a new record is created in the historical display grid is if the question is answered differently in a subsequent OASIS assessment. |
To view existing herpes zoster/shingles vaccine information for a patient:
1. If necessary, select the patient that you wish to add vaccination information.
2. Navigate to Patients | Intake/Referral | Health Screening.
3. Existing vaccination records can be viewed in the grid at the top of the screen, which utilizes the following columns of data:
To enter new herpes zoster/shingles vaccine information for a patient:
1. If necessary, select the patient that you wish to add vaccination information.
2. Navigate to Patients | Intake/Referral | Health Screening.
3. In the Herpes Zoster/Shingles Vaccine section, answer the following 'yes' or 'no' question: "At Home Health Start of Care, did the patient report ever having received the Herpes Zoster/Shingles Vaccine? ":
● If Yes is selected, simply click to complete the vaccination entry.
● If No is selected, continue with step 4.
4. In the Vaccine offered section, select the appropriate radio button to indicate whether or not the patient received a vaccination.
5. If the employee accepted and received an influenza vaccination by the agency, select the Vaccine offered and received radio button and then enter the date of vaccination in the Date field. Proceed by skipping to step 9.
6. If the employee declined an influenza vaccination by the agency, select the Vaccine offered and declined radio button.
7. Using the Reason Declined menu, specify the appropriate response(s) from the options listed:
● Proof provided vaccine received outside of HHA
● Allergy
● Compromised immune system
● Financial reasons
● Lack of access to the vaccine
● Other medical illness, etc.
● Spiritual or religious beliefs
● No reason provided
● Additional reason provided

Multiple options can be selected in the Reason Declined menu by holding down the CTRL key (on the keyboard) while making selections. |
8. If necessary, enter additional information for selection(s) made in the Reason Declined menu:
● If "Proof provided vaccine received outside of HHA" is selected, enter the date of vaccination in the Date field.
● If "Additional reason provided" is selected as the Reason Declined, enter any applicable Comments in the text field.
9. Click to save the new vaccination record for the patient.
10. Review the new entry in the display grid to ensure data was entered correctly. If any information is incorrect, select the corresponding entry in the display grid, click Delete, and then repeat steps 3-9 for the vaccination record.

Vaccination records that are entered using HCF comprehensive OASIS documents cannot be deleted in Health Screening. These records can only be removed from the display grid if the associated OASIS document is deleted (in Patients | Documents). |
